AUSTRALIA in the 1960s was a different country: poorer, less educated, closed markets, closed minds. We also had a disgraceful health system. Doctors, like hairdressers, were free to charge what they liked. And governments let them. People took out private health insurance to cover the cost of GP visits. And if you couldn't afford the insurance, you asked the doctor for charity.
